This survey is designed for council staff to submit paper and easy read responses to their resident survey on behalf of their residents.

How residents' data will be used

Data Cymru is running this survey on behalf of local authorities in Wales. Information that could identify residents will be seen by their council and Data Cymru only. This information will be held for 3 years from the date the survey closes in line with the requirements of the Data Protection Act 2018 and the UK General Data Protection Regulations (UK GDPR). Your IP address will be collected to help identify duplicate or inappropriate responses.

Completing the survey

You may fill in the survey either manually by copying out the individual paper responses, or by uploading a bulk excel file if you have already aggregated responses. A file will be provided for you to use as a template. If you choose to upload a file it must be in .xls or .xlsx format.
